Dan,

Yes you should file a new Amended return.  In the part III (reason for change) of the form of  provide your explanation of changes AND also advise the IRS to see a copy of their notice  (attach a copy to the amended return)

Dan,

It all depends if your tax refund / amount due was adjusted by the FIRST amended return.  If so compute the new amended return using the adjusted numbers from the First amendment.  If not then use the Original tax return numbers as your starting numbers
