Dear Seth, It was a pleasure helping you with your Facebook issue today. Here is a summary of the resolution.
Issue identified :

Account disabled

Solution Implemented :

Call customer / Remote Access

Account is disabled permanently and no appeal in the warning page.

Advised customer that he can send an appeal thru the help center but it is minimal to no response or ever getting the account back.

Advised to request an email address removal from the account and customer will just create another account
